Chronic Care Model
125 million Americans are affected by Chronic Disease. The direct cost of caring for people with chronic conditions is over $510 billion per year, which is 60% of the nation’s health care costs. The Chronic Care Model, developed by Dr. Ed Wagner with support from the Robert Wood Johnson Foundation, includes six elements: community resources, organization of the health system, self-management support, delivery system design, decision support, and clinical information systems. The goal is to use our knowledge of common chronic illnesses to make predictable health improvements.
