Who will benefit from this individualized service: Anyone requiring anticoagulation therapy with warfarin or Coumadin™ for conditions such as Acute Myocardial Infarction, Atrial Fibrillation, Chronic Heart Failure, Ischemic Stroke, Pulmonary Embolism, Valvular Heart Disease, or Venous Thromboembolism (“Blood Clot”).
Service goals: Provide a convenient and consistent method of managing anticoagulation therapy to ensure appropriate dosing, monitoring, education, and avoidance of complications.
