Who will benefit from this individualized service: Adults diagnosed with high LDL cholesterol (“Bad Cholesterol”), high Triglycerides, or low HDL cholesterol (“Good Cholesterol”).
Service goals: Assist providers in the management and monitoring of high cholesterol in order to prevent cardiovascular events and adverse drug reactions. Assist patients to: 1) Improve understanding of cholesterol disorders, cardiovascular disease, and medication therapies; 2) Incorporate lifestyle changes into their daily lives; 3) Achieve optimal cholesterol levels as set forth in the National Cholesterol Education Panel Adult Treatment Program III; 4) Derive maximal benefit from cholesterol medications and prevent adverse drug reactions; and 5) Improve adherence to prescribed cholesterol treatment regimens.
