Chronic Care Model

125 million Americans are affected by Chronic Disease.  The direct cost of caring for people with chronic conditions is over $510 billion per year, which is 60% of the nation’s health care costs.

“Improving Chronic Illness Care” is a program of the Robert Wood Johnson Foundation housed at the MacColl Institute for Health Care Innovation in Seattle, WA.  There Ed Wagner, MD developed The Chronic Care Model, which is a model of care to help the health care system deliver services that can meet predictable health care needs among groups of patients with common chronic illness.

This is accomplished by making key changes in the six elements of The Chronic Care Model, which foster productive interactions between informed patients and a prepared healthcare team that lead to improved clinical outcomes.  The six elements of the model are Community Resources, Organization of the Health System, Self-Management Support, Delivery System Design, Decision Support, and Clinical Information Systems.
    
Family HealthCare Center has implemented The Chronic Care Model in the treatment of patients with diabetes, asthma, cardiovascular disease and depression since 2000, and has incorporated the same system of care in providing preventative services to its patients.
