Who will benefit from this individualized service: Anyone diagnosed with Diabetes, a chronic disease characterized by uncontrolled blood sugar.
Service goals: Assist providers and patients to: 1) Manage and monitor diabetes in order to prevent cardiovascular, kidney, and nervous system disease; 2) Control or reduce the impact of diabetes on daily life; 3) Reduce the occurrence and/or progression of long-term complications; and 4) Encourage active involvement in daily self-care.
