Who will benefit from this individualized service: People with chronic illness such as asthma, chronic heart failure, COPD, coronary artery disease, diabetes, hypertension, and those with complex medication regimens.
Service goals: 1) Perform a comprehensive medication review to identify, resolve, and prevent medication-related problems, including adverse events; 2) Optimizing medication therapy to achieve target goals as defined by national guidelines; 3) Monitor and evaluate response to therapy, including safety and effectiveness; and 4) Document and communicate essential information to the patient’s healthcare providers.
