Who will benefit from this individualized service: Adults diagnosed with high blood pressure or pre-hypertension.
Service goals: Assist providers in the management and monitoring of blood pressure to reduce the risk of cardiovascular and kidney disease. Assist patients to: 1) Improve understanding of blood pressure management, cardiovascular disease, and medication therapies; 2) Incorporate lifestyle changes into their daily lives; 3) Achieve optimal blood pressure as set forth in JNC VII, the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure; 4) Derive maximal benefit from available blood pressure lowering medications and prevent adverse drug reactions; and 5) Improve adherence to prescribed blood pressure lowering treatment regimens.
