Lifestyle medicine is incorporated into every aspect of our care.
Our approach is patient focused, which means you will have an active role in your own health care plan. The relationship between you and your provider is interactive and highly productive. It’s is an innovative medical model, based on the most up-to-date research.
Family HealthCare Center’s Chronic Care Model has been used to treat diabetes, asthma, cardiovascular disease and depression since 2000, and also plays an important role in disease prevention. The model relies on close cooperation between provider, patient, and the community to improve overall health and well-being.
One of the most important aspects of FHC’s approach is the Planned Care model, which helps our staff track, monitor and follow up on patients’ health goals and outcomes. Lifestyle Medicine, includes four elements that often help reduce chronic disease: nutrition, physical activity, tobacco cessation, and stress management.
FHC’s licensed registered dietician works closely with medical providers. Nutrition Counseling can improve well-being for patients suffering from a wide range of digestive disorders such as Chron’s disease, celiac disease, irritable bowel syndrome, and food allergies. Proper nutrition also plays an important role in correcting conditions like hypertension, high cholesterol, cardiovascular disease and weight problems.
Diabetes Education is one area where Family HealthCare Center has seen significant improvement in patients’ health and well-being. Certified diabetes education provides knowledge, skills, and tools that help patients manage their disease and avoid dangerous and expensive complications.
Family HealthCare Center also facilitates counseling services for patients struggling with depression, anxiety, substance dependency, or other psychosocial needs through referrals in a collaborative with Southeast Human Services.
