Sliding Fee Scale (SFS) Program:
This Program applies a discount to the cost of services, allowing patients to access affordable quality health, dental, and prescription services. Qualification for a discount is based on household size and gross income. The SFS applies to services provided by Family HealthCare Center providers only.
Family HealthCare Center provides patients interested in or applying for SFS status a handout explaining the eligibility requirements, procedures, and rules that must be met and maintained. Please stop by our main office to apply for the SFS program.
